WHAT WE ARE LOOKING FOR
Jefferson County’s Comprehensive Community Services (CCS) program within the Human Services department provides a flexible array of individualized community-based psycho-social rehabilitation services for consumers with mental health and/or substance use disorders across their lifespan. The program is in need of a Psychosocial Rehabilitation Worker to join the CCS Adult team in order to facilitate skill development with consumers who have mental health and/or substance use disorders.

WHAT YOU WILL DO
As a compassionate and attentive Psychosocial Rehabilitation Worker you will work with consumers in reaching their goals while preventing acute episodes and hospitalization by taking on the below essential responsibilities.

SOME ESSENTIAL RESPONSIBILITIES            
You will be successful in this role by completing the following tasks and responsibilities:

• Assists consumers, 18 years of age and older, with learning and practicing skills and identifying values.
• Establish relationships with natural supports, and community supports to assist in skill development to help the consumer be successful at home, school, work, and/or the community.  
• Works with consumers to increase independence through mentoring and educating consumers in developing independent living skills and values.  
• Provides support to assist service facilitators and supervisors in assessing consumers in treatment; provides service planning and review goals.
• Assists in coordinating groups and needed services and provides appropriate referrals to available resources including legal, academic, medical, housing, community outreach programs, and multi-disciplinary county programs. 

 

Requirements:

• High school diploma or equivalent

• Valid Drivers License.

Preferred Requirements:

• Associate degree in Social Work, Human Services, or Social Sciences and one year experience or an equivalent combination of education and experience.
• Bachelors degree in Social Work, Human Services, or Social Sciences or an equivalent combination of education and experience.
• Applicants with previous or current Peer Support Specialist experience encouraged to apply.
